1956 – 1960

Even with the introduction of signals at railroad crossings, accidents continued to occur. In 1972 the average number of collisions at U.S. highway-rail grade crossings had risen above 12,000 incidents yearly. Idaho launched a campaign called Operation Lifesaver to promote highway-rail grade crossing safety, which decreased accidents by 43% in a year.

The U.S. Interstate Commerce Commission and Bureau of Safety undertook investigation and analysis of railroad accidents in 1901. In 1967, the National Transportation Safety Board was established. It is responsible for determining the probable cause of civil transportation accidents.
