Completing the SF-428

SF-428 Tangible Personal Property Report

There are 4 versions of this form that are used by most grantees to report on equipment purchases. It can be confusing to understand which version of the form to use. Equipment is defined by 2 CFR 200 as durable personal property having an acquisition cost greater than or equal to $5,000. Example: You use your grant funding to purchase a car for $6,000. You would need to report on this car until its depreciated value falls below $5,000.
  • The SF-428 is an optional coversheet.
  • The SF-428A should be used when you are submitting an interim report.
  • The SF-428B should be used when you are submitting a final report.
  • The SF-428C may be submitted at any time during the life of the award when you are requesting permission to dispose of the equipment. This means that NPS must receive and approve this form prior to you disposing of the equipment.
If you have not purchased reportable equipment, then you do not need to submit any SF-428 series forms.
