You are required to complete and submit an annual CUA report per the conditions of your CUA. The annual report includes questions about visitor use statistics, reportable injury data, and gross receipts earned from operations in the NPS. Your annual CUA report is due early in the calendar year for the preceding operating year (e.g., report for 2025 will be due in early 2026). Additionally, some parks require you to submit a monthly CUA report that includes questions only about visitation statistics and reportable injury data.
The online CUA system provides a way for you to submit these required reports if your CUA was issued through that system and requires response to the questions found in the OMB-approved annual CUA report (NPS Form 10-660)(.pdf, 644KB) and monthly CUA report (NPS Form 10-660A)(.pdf, 584KB). Parks issuing CUAs outside of the online CUA system are required to use the OMB approved annual and monthly reporting forms.
For all issued CUAs, both through and outside of the online CUA system, parks are allowed to add information requests specific to the authorized service activity (e.g., how many times did you access a specific lake).
Failure to submit required reports or misrepresentation of reported statistics or earning may result in denial of your next CUA application.
