To submit a registration request:
1) Click the link below to access our online form. Fill out the form completely and fax it to the number indicated at the end of the form.
2) Please retain the printed copy for your records as confirmation of your submission. Be advised that you will not be able to save the contents of the form as an electronic file.
- Be sure to register for a workshop if you are not workshop qualified.
- If you are willing to accept other dates than those you select, please check that box on the form. We will contact you by email to verify that the new date is acceptable before faxing your confirmation letter.
What happens next?
1) A confirmation letter will be faxed to the number you indicate within three weeks. This confirmation will provide the status of your request.
2) Have your principal sign your confirmation letter and fax it back to the number indicated to complete your registration!
