Description: The transitions program is a component of the YouthNet Center based in Chicago. Its programming provides at-risk youth recreation programs, self-awareness, anti-gang, crime, and anti-drug programs. The program is designed to help youth stay away from anti-social behavior activities, improve theirselves through recreation programs and self improvement activities. The program is administered by the Westside schools, and Communities Organizing for Restructuring and Planning (a non-profit organization).
