SF 424 Application for Federal Assistance
FY2012 JACS Grant Project Proposal
“Certifications Regarding Debarment, Suspension and Other Responsibility Matters, Drug-Free Workplace Requirements and Lobbying" (DI-2010) (PDF)
• Proof of applicant's governmental, nonprofit, or institutional status
• Letter from property owner giving consent, if applicable
• Federally Approved Indirect Cost Rate Agreement, if applicable

Non-Construction projects must also submit:
